چکیده

Background: Impaired insight is a common feature of schizophrenia, related to poor treatment compliance and unfavorable outcome. This study was aimed to explore the internal structures of insight evaluated by self-reported and clinician-rated measures. Methods: To measure insight into psychosis as a pilot study, Korean versions of VAGUS Self-Report(VAGUS-SR), ClinicianRated(VAGUS-CR), The Scale for the Assessment of Unawareness of Mental Disorder(SUMD), and Schedule for the Assessment of Insight (SAI) were evaluated in 41 patients with schizophrenia. Multivariate statistical analyses were used to examine insight structures. Results: VAGUS-SR and VAGUS-CR were significantly correlated to each other(r = 0.73, p < .001), as well as SUMD and SAI. Gender of patients has a significant effect on the subscale scores. The link between ‘illness awareness’ and ‘need for treatment’ was revealed in VAGUS-SR, in contrast to VAGUS-CR which the link between ‘illness awareness’ and ‘symptom attribution’ was observed. Discussion: This study demonstrated that insight measured by self-report and clinician-ratings revealed distinct internal structures. The difficulties in communication between clinicians and patients may affect differences in insight structures. The VAGUS-CR and VAGUS-SR might be used complementarily to gain detailed information on insight into psychosis.

Sarcosine or D-serine add-on treatment for acute exacerbation of schizophrenia: a randomized, double-blind, placebo-controlled study.

CONTEXT Agents that enhance N-methyl-D-aspartate (NMDA) function through the glycine modulatory site (D-serine, glycine, or D-cycloserine) or through glycine transporter 1 (sarcosine) improve the symptoms of patients with stable chronic schizophrenia. OBJECTIVE To determine whether NMDA-glycine site agonists or glycine transporter-1 inhibitors have better efficacy and whether NMDA receptor-en...

Add-on treatment of benzoate for schizophrenia: a randomized, double-blind, placebo-controlled trial of D-amino acid oxidase inhibitor.

IMPORTANCE In addition to dopaminergic hyperactivity, hypofunction of the N-methyl-d-aspartate receptor (NMDAR) has an important role in the pathophysiology of schizophrenia. Enhancing NMDAR-mediated neurotransmission is considered a novel treatment approach. To date, several trials on adjuvant NMDA-enhancing agents have revealed beneficial, but limited, efficacy for positive and negative sympt...

Glycine Site Modulators and Glycine Transporter-1 Inhibitors as Novel Therapeutic Targets for the Treatment of Schizophrenia

Current antipsychotic medications are efficacious for the positive symptoms of schizophrenia. However, there remains a significant unmet need for alternate strategies that could result in improved tolerability and/or efficacy for negative and cognitive symptoms. A growing body of research suggests that NMDA mediated neuronal activity is involved in the etiology of schizophrenia. Glycine binds t...

D-cycloserine in Schizophrenia: New Strategies for Improving Clinical Outcomes by Enhancing Plasticity

BACKGROUND Dysregulation of N-methyl D-aspartate (NMDA) receptor signaling is strongly implicated in schizophrenia. Based on the ketamine model of NMDA receptor hypoactivity, therapeutic approaches designed to maintain a sustained increase in agonist activity at the glycine site of the NMDA receptor have produced promising, although inconsistent, efficacy for negative symptoms. METHODS A revi...

The Effectiveness of Cognitive Rehabilitation on Improving Cognitive Deficits (Attention and Memory) in Patients With Chronic Schizophrenia Based on Cognitive Levels

Introduction: Cognitive deficits have been recognized as a core feature of schizophrenia, which is directly associated with the functional and social outcomes of this disorder. Cognitive remediation therapy aims to improve the deficits and the following outcomes. The purpose of this study was to investigate the effectiveness of cognitive remediation therapy on attention and working memory in tw...
